Care is planned and provided by a team of individuals with expertise in a variety of areas. The Charise Hospice Care Team consists of the following professionals and services:
Your Attending Physician
The patient’s primary doctor continues to oversee care and works closely with the hospice team to ensure proper treatment decisions.
Hospice Medical Directors
Specialized doctors manage and supervise the hospice care plan and ensure comfort-focused treatment and symptom control.
RNs on Call 24/7
Registered nurses are available around the clock to provide support, guidance, and respond to emergencies whenever needed.
Hospice Nurses
Hospice nurses regularly visit patients to monitor health, manage symptoms, and provide medical care and family education.
Hospice Aide
Assist patients with daily personal care such as bathing, dressing, grooming, and maintaining hygiene and comfort.
Social Worker
Provide emotional support, counseling, and help families access community resources and cope with difficult situations.
Chaplains / Spiritual Support
Offer spiritual guidance, emotional care, and support aligned with the patient’s beliefs and values.
Trained Volunteers
Provide companionship, emotional support, and help reduce feelings of loneliness for patients and families.

Where Is Hospice Care Provided?
Hospice care is available wherever the patient is most
comfortable:
- Patient, family or caregiver homes
- Hospitals, skilled nursing facilities
Who can make a referral?
Anyone Referrals are made by family, friends, clergy, physicians and staff at health care facilities. Patients may call us themselves. We receive as many referrals from family members as we do from hospitals and facilities.
How is hospice paid for?
Hospice is covered under Medicare, Medicaid, most private insurance plans, HMOs and other managed care organizations. No one is denied care due to inability to pay.
What We do
We care for patients, families and caregivers as early as possible in the late-stage disease process. Our team of doctors, nurses, social workers, spiritual counselors, hospice aides and volunteers works collaboratively to ensure comfort, dignity and quality of life for the entire family as long as possible
Home Hospice
- The most traditional form of hospice care.
- Allows patient to remain in the comfort of their own home.
Continuous Care
- Nurses stay in the home until the symptoms are controlled.
- Need for continuous care is evaluated daily.
